As the patient it is your responsibility to be aware of all aspects of your health care plan, including co-pay amounts, covered services, deductible amounts, etc. If your current health insurance plan is one in which we participate, applicable deductibles, co-payments, percentages, and non-covered services will be collected at the time of service. Otherwise, payment in full is required for ALL services at the time they are rendered.
We accept payment in the form of VISA, MasterCard, cash, or personal check. There is a $35.00 charge for insufficient checks.
